Acute Respiratory Distress Syndrome
Acute Respiratory Distress Syndrome:
This session is designed to familiarize participants with management and support of the patient with Acute Lung Injury (ALI) and Acute Respiratory Distress Syndrome (ARDS). Discussion will include pathophysiology in ALI/ARDS as well as current support strategies such as conventional and advanced mechanical ventilation, positioning and pharmacologic therapy.
Objectives:
1. Participants will be able to describe the pathophysiologic changes at the alveolar level that prevent normal ventilation and gas exchange.
2. Participants will be able to describe the two principles of lung protective ventilation strategies
3. Participants will be able to describe the non-ventilation support strategies used in ARDS
Questions:
• Is there a difference in complexity and outcome in ARDS patients depending on the type of injury to the lung?
• What are the basic strategies when ventilating a patient with ARDS?
• Is any mode of ventilation shown to be superior in ventilating patients with ARDS?
• What is the role of pulmonary vasodilators in management of the patient with ARDS?
• Why is mortality lower in ARDS patients today then when it was discovered 30 years ago?
• Are steroids helpful in ARDS?
• Is prone positioning still beneficial?
Cardiac Dysrhythmias Made Easy:
Cardiac rhythms are now being monitored across the continuum of care in many different patient settings. Yet sometimes it's difficult to differentiate similar rhythms or identify infrequently seen but potentially life threatening dysrhythmias. This session will focus on significant rhythms and their implications for nurses. You'll learn simple techniques to help identify dysrhythmias and nursing implications associated with each will be presented. The session is appropriate for nurses in all practice settings.
Learner Outcomes:
Discuss the electrophysiology of the cardiac conduction system.
Identify common dysrhythmias and the nursing implication for each.
Identify lethal dysrhythmias and the nursing implications for each.
Fluid Balance and Resuscitation
Fluid Balance and Resuscitation:
“Fluid Balance and Resuscitation (Burn and Trauma Patients)”
Fluid resuscitation is at the cornerstone of management of the burn and trauma patient. Trauma patients who have sustained massive injury and hemorrhage must receive fluid replacement to prevent hypovolemia and ensuing shock. For the burn patient, fluid resuscitation is critical throughout the first 24 hours after the injury is sustained to prevent hypovolemia and ensure adequate tissue perfusion. Delayed or inadequate fluid resuscitation results in suboptimal tissue perfusion, which can lead to multisystem organ failure and death. Overresuscitation can be more problematic than underresuscitation and has been associated with the development of abdominal compartment syndrome, compartment syndrome of the extremities, airway obstruction, and pulmonary edema. The term fluid creep is used to describe the tendency to give too much fluid and can result from the hemodynamic consequence of opioid creep. Experts in the field agree that fluid resuscitation is a priority. The advanced practice nurse (APN) can play a vital role in implementing strategies to ensure optimal resuscitation in the burn and trauma patient. Through collaboration, the APN in both the burn center and the emergency department must make certain that the educational needs of the staff are addressed and be diligent in providing significant education, essential tools, and assistance to the staff nurses in an effort to promote best-practice and evidence-based care.
Questions:
What are the advantages of administering lactated Ringers when considerable amounts of fluid are needed during resuscitation?
How does the administration of opioid analgesics affect the resuscitative efforts of the burn or trauma patient?
What are the deleterious effects of over-resuscitation or fluid creep?
While utilizing the Parkland Formula, what is the most common error made by clinicians when calculating fluid needs of the burn patient?
Managing Alcohol Withdrawal Syndrome
Over 10 million Americans have a problem with alcohol dependency and 40% of hospitalized patients are at risk for alcohol withdrawal syndrome (AWS). This session will focus on the risk factors and signs and symptoms for acute AWS. Learn how to manage your patient's withdrawal symptoms and keep yourself and the patient safe throughout the process.
Learner Outcomes:
Identify the etiology and pathophysiology of alcohol withdrawal syndrome.
Screen patients and identify appropriate diagnostic studies for patients at risk for AWS.
Recognize and manage symptoms of AWS using pharmacologic and nonpharmacologic therapies.
Strategies for Reducing Overcrowding the Emergency Department
Description: This online session will focus on factors that contribute to ED overcrowding using a cardiac conceptual model for ED operations. Equating preload to the volume of patients coming into the ED, afterload as the resistance to flow out of the ED, and contractility and the essential work performance of staff in the ED, pathologic issues that can negatively impact ED function and outcomes will be discussed. Strategies that ED leaders can employ to effectively treat these issues will be offered.
Questions:
1. What are key factors that lead to ED overcrowding conditions?
2. What ED metrics can be used to identify specific causes of crowding?
3. How can ED and hospital leaders collaborate to address ED overcrowding effectively?
4. What are short and long-term strategies that can have a positive impact on reducing crowding?
Summer Emergencies: Lightning, Spiders, & Snakes, Oh My!
Summer's almost here- and so are the thunderstorms, bugs and hot, humid weather. As people emerge to enjoy outdoor pursuits, be prepared- some of the most innocuous activities have unexpected risks, including lightning strike, snake and insect bites, and heat-related illnesses. This presentation will describe immediate life-saving interventions and offer common-sense prevention strategies for nurses to incorporate into their own lifestyle and health teaching opportunities.
Learner Outcomes:
- Identify hazards that pose a risk to humans in the outdoor summer environment.
- Learn management techniques for individuals who sustain environmental-related injuries and illnesses.
- Identify effective prevention strategies to avoid or minimize summer environmental hazards.
